Your first visit with the Hobson Institute marks the beginning of your healthcare journey with us. We offer in-person and virtual appointments based on your needs. To optimize your experience, we kindly ask you to complete the following forms fully and correctly. It assists us in processing the necessary paperwork we need to ensure that our services are seamlessly tailored to your needs.

We are happy to bill your insurance for out-of-network benefits and will work with you any way we can. I think we need to reword this as we have stopped submitting for out of network benefits not too long ago. “You will be charged at an out of network rate. We are more than happy to provide you with a superbill should you choose to submit to your insurance for your out of network benefits.”
A licensed physical therapist will go over your patient history and perform a bio-mechanical evaluation of the problematic and surrounding areas. Of course every case is different and unique. Our evaluation will typically include postural assessment, breathing evaluation, range of motion of the specific joints and surrounding areas and joint mobility testing, strength, neurological testing, gait and balance testing.
If you do not have your doctor’s referral, you can still undergo the evaluation without a problem. Illinois is a direct access state which means that one can come to physical therapy directly without a doctor’s referral. That being said, many insurance plans still require a doctor’s referral. Therefore, we still require receiving your referral from your doctor or can request a referral from your primary physician, family doctor, OBGYN, chiropractor, dentist, or oral surgeon if you don’t have one.
You should come 15 minutes before your scheduled appointment to be able to fill out any paperwork and to go over initial details. Typically we send an welcome email that includes the forms that will be required for registration. If you forget the forms, then we will be able to provide them for you at the office. It is always a good idea to come early as registration will take a little time and we don’t want you to miss out on any time with your physical therapist.
Your physical therapy sessions will last about 45 minutes to an hour. Each session will include one-on-one care with your physical therapist. Treatments normally include a manual therapy portion where the physical therapist will use their hand- on skills to assist the healing process. Depending on your condition, you will also be expected to perform exercises to help strengthen your body and work on correct posture and breathing.
Please wear comfortable, loose fitting clothing. Masks are optional in our office.
