Your first visit with the Hobson Institute marks the beginning of your healthcare journey with us. We offer in-person and virtual appointments based on your needs. To optimize your experience, we kindly ask you to complete the following forms fully and correctly. It assists us in processing the necessary paperwork we need to ensure that our services are seamlessly tailored to your needs.

General Questions

Hobson Institute is contracted with some medical insurance companies through our “Physical Therapy Renaissance DBA Hobson Institute In Network” sister practice. We are in a network with BlueCross BlueShield PPO plans and Medicare. Please call 312-986-9833 for more information.

We are happy to bill your insurance for out-of-network benefits and will work with you any way we can. I think we need to reword this as we have stopped submitting for out of network benefits not too long ago. “You will be charged at an out of network rate. We are more than happy to provide you with a superbill should you choose to submit to your insurance for your out of network benefits.” 

A licensed physical therapist will go over your patient history and perform a bio-mechanical evaluation of the problematic and surrounding areas. Of course every case is different and unique. Our evaluation will typically include postural assessment, breathing evaluation, range of motion of the specific joints and surrounding areas and joint mobility testing, strength, neurological testing, gait and balance testing.

If you do not have your doctor’s referral, you can still undergo the evaluation without a problem. Illinois is a direct access state which means that one can come to physical therapy directly without a doctor’s referral.  That being said, many insurance plans still require a doctor’s referral.  Therefore, we still require receiving your referral from your doctor or can request a referral from your primary physician, family doctor, OBGYN, chiropractor, dentist, or oral surgeon if you don’t have one.

You should come 15 minutes before your scheduled appointment to be able to fill out any paperwork and to go over initial details.  Typically we send an welcome email that includes the forms that will be required for registration.  If you forget the forms, then we will be able to provide them for you at the office.  It is always a good idea to come early as registration will take a little time and we don’t want you to miss out on any time with your physical therapist.

Your physical therapy sessions will last about 45 minutes to an hour.  Each session will include one-on-one care with your physical therapist.  Treatments normally include a manual therapy portion where the physical therapist will use their hand- on skills to assist the healing process. Depending on your condition, you will also be expected to perform exercises to help strengthen your body and work on correct posture and breathing.

Please wear comfortable, loose fitting clothing. Masks are optional in our office.

  • Lincoln Park Clinic:
    • The Lincoln Park location is in a residential area with plenty of street parking adjacent to the clinic on the side streets before 6pm without any cost or permit.
  • Loop/Downtown Clinic:
    • Parking is always a challenge in downtown Chicago. If you are driving to your appointment, we recommend the one of following options:
      • Public Transit
        • The office is accessible by bus and train (Redline: State/Lake or Brown line: Washington/Wabash).
      • Valet Parking with “New York Jewelers” 
        •  9 S. Wabash Street. Located on Wabash, half a block south of Madison and Wabash (1 block west of Michigan Ave). This parking is $16 for 0-2 hours, or $22 for 2-5 hours CASH only.
      • Valet Parking with “The Jewelers Center” 
        • 19 S. Wabash Street. Located on Wabash, half a block south of Monroe and Wabash (1 block west of Michigan Ave). This parking is $16 for 0-2 hours, or $22 for 2-5 hours CASH only.
      • Street Parking 
        • Use the parking website Interpark for the self park on Adams and Wabash or the SpotHero App to reserve parking.
  • If you have a co-payment, your credit or debit card on file will be charged each visit upon check in, according to the policies of your insurance company. If you have a deductible, you may be asked to pay each visit, or you will receive a statement by email, which is due upon receipt.  If you have co-insurance, you will receive a statement, as well. If you are still attending appointments and didn’t have an opportunity to pay your statement, your patient care coordinator will prepare a statement.  Payment will be charged to the card on file at that time. If at any time you wish to present a different form of payment, please ask your patient care coordinator.  
  • If you have a health savings account, we are happy to provide any documentation required by your plan.  We will attempt to run the charges on any health savings card you present, but please understand that many cards may be declined due to the funds not yet being available or other limitations of the health savings card.  In that case, we request another form of payment and will provide you with the documentation you need to request reimbursement from your plan
  • While all patients must maintain a card on file, we also accept CareCredit and personal checks for payment.  Please call (312) 986-9833 with any questions or for more information.
